Porting Request Form

Please follow the link below to port your number(s) into your existing portal hosted with Sure Communication.

You will need the following;

• The number you wish to port in
• The destination number of the number you wish to port in
• The details of the provider your number currently resides
• A digital copy of the latest bill* for your number, scanned and uploaded
• A copy of this form* (please download) completed, signed, scanned and uploaded

Please scan and create an image or PDF as you will be able to upload during the Porting Wizard process.
